This is only an option for very large breasted women. In these cases, a larger lumpectomy combined with a variety of oncoplastic techniques removes the tumor or lesion, but also some normal breast tissue for a nicer cosmetic result. These are often done in combination with a reduction on the other side to improve symmetry.
